We are honored to be continually recognized by the NCQA (National Committee for Quality Assurance) as a “Patient-Centered Medical Home” practice since 2014.
The patient-centered medical home is a model of care that puts patients at the forefront of care. It fosters to build better relationships between patients and the clinical care teams.
Research shows that PCMHs meet the Triple Aim of Healthcare. It improves the quality of care and the patient experience, increase staff satisfaction and reduces health care costs.
We, as a PCMH certified practice, is committed to continuous quality improvement and a patient-centered approach to care.
Meet The Team
Clinical

Renante Ignacio, MD
He graduated medical school in 1994 from the University of Santo Tomas, Manila Philippines and migrated to the country and completed his Internal Medicine Residency in 1999. He further pursued training and completed a two-year Geriatric Fellowship and graduated in 2001 and since then has been a full time Geriatrician in mostly the post-acute and long-term care settings. He remains certified in three specialties, namely Internal Medicine, Geriatric Medicine and Hospice and Palliative Care Medicine. He worked for Elder Medical Services, PC prior to establishing Geriatric & Palliative Specialist, PC in October 2012. He has served as a Medical Director of various Skilled Nursing and Assisted Living Facilities in the Central New York Area since 2002. He also serves as a Clinical Assistant Professor at SUNY Upstate Medical University at the Department of Geriatric Medicine since 07/01/23. To further promote and foster the care of the frail elders, he remains involved as a Board Member in various national, state and local health organizations. He loves swimming, diving and any water related activities and certainly makes time for traveling with friends and family in his spare time.

Beverly Chartrand, FNP
She has been a RN for 40 years and a Nurse Practitioner for 9 years. She received a Bachelor of Science in Nursing in 1983 from James Madison University with magna cum laude honors and a Family Nurse Practitioner/Master’s Degree Program with an Education Minor from Upstate Medical University in 2015 with summa cum laude in 2015. She joined our team as a Nurse Practitioner in May 2015, and has worked with Dr. Ignacio providing comprehensive health care to all of our residents in assisted living and residential care facilities. She lives in Minetto with her husband. Her favorite pastime is spending time with her 4 children and 7 grandchildren, and her favorite place to be is boating in the Thousand Islands or spending time at the ocean.

Sara Domachowske, Nurse Practitioner
She received a Bachelors in Biomedical Sciences with a Minor in Chemistry from SUNY Cortland, Associates in Sciences for Registered Nursing from Cayuga Community College, Bachelors in Sciences for Registered Nursing from University of Phoenix, and Masters in Sciences for Family Nurse Practitioner from SUNY Polytechnic Institute. She joined our team as a Nurse Practitioner in August 2023. She started her career as a CNA, then became an RN in the Intensive Care Unit, Surgical Services, and Gastrointestinal/Endoscopy specialty unit. She is dedicated to improving the quality of life for patients and creating a positive atmosphere across the healthcare continuum. During her time at home, she and her husband take care of their 4 young children; 3 boys and 1 girl and a dog in a high energy household. They spend a lot of time playing outside, having campfires, going for walks to get ice cream, and coming up with new things to keep themselves busy.

Billie-Jo Ramachandran, Nurse Practitioner
She received an Associates in Applied Science in Nursing in 2016 from Cayuga Community College with Cum Laude honors and completed the bridge program through Upstate Medical University in 2018. She completed the Family Nurse Practitioner/Master's Degree Program with honors from Upstate Medical University in 2022. She is board certified as a Family Nurse Practitioner through the ANCC (American Nurses Credentialing Center) since September 2022. She has previously worked as a Nursing assistant in the U.S. Air Force, a Medical/Surgical floor RN at Oswego Hospital and Inpatient Psychiatric RN at Lobdell Center for Mental Health and Wellness. While studying for her Nurse Practitioner’s degree she worked and continued to be employed as a floor RN at Lobdell Center for Mental Health and Wellness at Lakeview. She joined our team as a Nurse Practitioner in October 2022. She enjoys reading, listening to audio books, traveling, and most of all spending time with family.

Baljeet Khera, Nurse Practitioner
She received a Bachelors in Biomedical Sciences with a Minor in Chemistry from SUNY Cortland, Associates in Sciences for Registered Nursing from Cayuga Community College, Bachelors in Sciences for Registered Nursing from University of Phoenix, and Masters in Sciences for Family Nurse Practitioner from SUNY Polytechnic Institute. She has previously worked as a Medical/Surgical floor RN at Oswego Hospital and Hospitalist NP and currently works as a Psychiatric NP at Lobdell Center for Mental Health and Wellness. She joined our team as a Nurse Practitioner in June 2022. She enjoys listening to Indian classical music, traveling to different countries to explore and experience different cultures.

Mary Ann Madlangbayan, RN BSN
She received a Bachelor of Science in Nursing in 1992 from the Philippines. She has worked in three different countries as a Registered Nurse since 1992. She started as an RN at a Skilled Nursing Facility in New York City when she migrated to the United States from 2006-2009. She then moved to Oswego, NY and worked as a Medical/Surgical floor RN at Oswego Hospital, then assumed the role of Clinical and Quality Coordinator at Geriatric & Palliative Specialist from 2012 and currently works as a Home Care RN case manager. She currently helps triage and attends to off hours phone calls for the group.

Theresa Wetzel, Medical Assistant
She has been working as a medical assistant in the long term care field since 2007. She started her career with Geriatric and Palliative care in April 2020. She loves her job and enjoys all aspects. She enjoys camping, wood burning and other crafts and loves spending time with her family and fur babies.

Mariah Moshier-Feeney, Medical Assistant
She graduated from Center for Instruction Technology & Innovations (Citi) with certification of Medical Assistance in 2015. She previously worked in the Oswego Hospital Emergency Department as a technician from 2015-2017. She has been working with Geriatric & Palliative Specialist, PC since 2017. She has expanded her knowledge in geriatric care in our local nursing and assisted living facilities over the years. She is married with two young boys and enjoys camping during the summer with her family. She also likes to travel around NYS and visit various state campgrounds. She also enjoys gardening and walking along the beautiful Lake Ontario, watching sunsets.

Michelle Pritchard, Clinical Coordinator
She is a native of Oswego and has a background in Healthcare. She has been working at Geriatric & Palliative Specialist, PC for the past 10 years. She has worked on numerous initiatives aimed at enhancing the quality of care we provide. She is the main moving force for our group's continuing Patient-Centered Medical Home (PCMH) recognition and certification since 2019, a testament to her unwavering dedication to putting patients first. She enjoys event planning, traveling, and spending time with family.

Melissa Treasure, Office Manager
She started with our practice in 2013 as a Billing Specialist for 8 years prior to being promoted to the position of Office Manager in 2022. She performs quality assurance and handles any patient concerns or complaints. She lives in Oswego with her fiancè, son and 3 pugs. She also enjoys spending time with her family, camping, and gardening.

Fe Anne Ignacio, Administrative Assistant
She has a Bachelor of Arts Major in Behavioral Science in 1996 from the University of Santo Tomas, Manila, Philippines. She has experienced working in the HR Department in the hotel industry as well as in the Philippine government from 1996-2007. She has been working with the practice since 2014. She works as an Administrative Assistant who is responsible for the diverse set of administrative tasks that support the physician and other healthcare professionals in the practice. She is also in charge with the day-to-day duties required to keep the office runs smoothly. She loves to travel with her family and enjoys cooking and baking in her spare time.

Yolanda Ignacio, Secretary
She received a Bachelor of Science in Medical Technology degree in 1968 in Manila, Philippines. She worked as a Senior Medical Technologist and later as a Laboratory Supervisor at various offices and hospitals from 1974-1997 in the Philippines and Saudi Arabia. She started as a caregiver at St. Anne’s Residential in San Bruno, CA from 2001-2007 when she migrated to the United States. She later worked at Elder Medical Services, PC as Secretary to Dr. Ignacio from 2008-2012. She has been working at Geriatric & Palliative Specialist as a secretary in 2012 up to present time. She is primarily involved in the preparation of Care Plans for all our patients. She loves to cook and do gardening in her spare time as well as traveling with her family.

Roman Joseph Madlangbayan, Office Assistant
He graduated from SUNY University at Buffalo with a Bachelor of Arts degree in Biological Sciences and decided to pursue further studies, attending Nursing School at Cayuga Community College in Fall 2024. He has been working at Geriatric & Palliative Specialist since 2019. He works as a Medical Office Assistant focusing on Chronic Care Management and Merit based Incentive Payment System (MIPS). He enjoys weightlifting in spare time, spending time with family and friends and learning new things in the medical world.

Emmanuel Ignacio, Office Assistant
He is an undergraduate student pursuing a Bachelor of Science in Health Science and Public Health at Stony Brook University graduating in 2025, with the hopes of attending medical school after. He has been working with Geriatric & Palliative Specialist since 2022. He works as a Medical Office Assistant supporting the clinical work of the physician and the rest of the healthcare team, as well as engaging with assisted living facility residents. He enjoys swimming in his spare time, spending time with family and friends, and traveling.

Rommel Madlangbayan
He joined Geriatric & Palliative Specialist, PC in 2012 and has been working with the practice performing clerical and custodial works. He received his Nursing Assistant Certification in November 2008 from New York City. He has been working as a CNA since 2010. He loves music and plays guitar in his spare time.

She joined as a billing specialist in March 2022. She has been a Medical Biller for nearly years. She lives in Oswego and enjoys traveling, reading, and going to concerts.

She has been a Medical Biller for over two years. She lives in Oswego with her husband but takes frequent trips to England to visit their daughter. She enjoys anything Disney related, traveling and spending time with their 2 dogs and 2 cats.
